Introduction and presentation
Burkitt (BL) and lymphoblastic lymphomas (LBL) are highly aggressive diseases with distinct natural histories and clinical presentations. BL mostly occurs in the first two decades of life and accounts for 1–2% of all lymphomas. Three clinical variants are recognized: (1) endemic BL, which is primarily found in equatorial Africa; (2) sporadic BL, which presents worldwide but is the most common type in western countries; and (3) immunodeficiency-associated BL, which is associated with HIV infection. There are important clinical differences in these variants (Table 11.1), with endemic BL involving the jaw, orbit, and paraspinal regions in half of the cases as well as the mesentery and gonads, while sporadic BL mostly involves the distal ileum, cecum, and/or mesentery, and rarely the jaw. When bulky or disseminated disease is present, extranodal involvement of the ovaries, kidney, breasts, and/or CNS may be seen. Clinical presentation in a Berlin–Frankfurt–Munster Group (BFM) series of 152 pediatric patients included advanced stage (III/IV) disease in 38%, bone marrow involvement in 33%, and central nervous system (CNS) disease in 4%. Of the patients in this series, 27% presented as acute leukemia, referred to as the L3 subtype of acute lymphoblastic leukemia (ALL) within the French-American-British (FAB) classification, or B-ALL. BL infrequently presents in adults, but does occur with increased frequency in patients with HIV infection.
LBL is most commonly a malignancy of T-cell precursor cells, and, as such, it is identical to T-cell acute lymphoblastic leukemia (T-ALL).

Significant new opportunities for astrophysics and cosmology have been identified at low radio frequencies. The Murchison Widefield Array is the first telescope in the southern hemisphere designed specifically to explore the low-frequency astronomical sky between 80 and 300 MHz with arcminute angular resolution and high survey efficiency. The telescope will enable new advances along four key science themes, including searching for redshifted 21-cm emission from the EoR in the early Universe; Galactic and extragalactic all-sky southern hemisphere surveys; time-domain astrophysics; and solar, heliospheric, and ionospheric science and space weather. The Murchison Widefield Array is located in Western Australia at the site of the planned Square Kilometre Array (SKA) low-band telescope and is the only low-frequency SKA precursor facility. In this paper, we review the performance properties of the Murchison Widefield Array and describe its primary scientific objectives.

Introduction
Since Köhler and Milstein first demonstrated that monoclonal antibodies (MoAb) directed against human differentiation antigens could be generated from hybridomas, which made large-scale production of MoAbs possible, there has been the expectation that these reagents would be used to treat hematologic malignancies. Subsequently, it was reported that fragments of antibody variable domains (Fv) could be linked together to make recombinant proteins capable of antigen binding. Methodologies in antibody engineering have since been developed to produce MoAbs and their fragments for clinical use and to generate constructs and conjugates with improved stability, more favorable pharmacokinetic profiles, increased affinity, enhanced effector functions, and reduced immunogenicity. This has facilitated the development of a large and growing number of MoAb-based agents that target antigens expressed by lymphohematopoietic cells for study and treatment in humans (Table 24.1). Leukemias are in general excellent candidates for MoAb-based targeted therapies because some antigens expressed on the surface of malignant cells are relatively lineage restricted and have limited normal tissue distribution (reviewed in Ch. 4). Since the late 1990s, significant progress has been made in the clinical application of antibody-targeted therapies in specific subtypes of leukemias and lymphomas in adults, leading to US Food and Drug Administration (FDA) approval for some of these novel drugs (Table 24.1). Many of the available MoAb-based agents that target lymphohematopoietic cell surface antigens have relevance to childhood leukemias, although at the time of writing this edition, only a small number of pediatric clinical trials have been conducted (reviewed in detail below). These studies offer proof of principle that MoAb-based therapeutics have the potential to overcome leukemia cell resistance to standard therapies and have relatively low risk of non-specific toxicities.

INTRODUCTION
Allogeneic Transplantation in Pediatrics
Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is curative for many pediatric diseases. Most children transplanted for cancer, severe combined immunodeficiency syndrome, aplastic anemia, sickle cell anemia, thalassemia, and certain metabolic disorders are expected to survive. This has led to an ever-increasing population of longterm survivors. Recent studies demonstrate the major impact that late effects have on the individual survivors and society as a whole.
Chronic Graft versus Host Disease
Chronic graft versus host disease (cGVHD) is the most significant nonrelapse cause of morbidity and mortality following stem cell transplantation (SCT) for malignancies. Although the rates of cGVHD are lower in children than adults, the incidence of cGVHD in children has increased in association with the use of peripheral blood and unrelated donors. The manifestations and mechanisms of cGVHD in children and adults appear similar, although the natural history and response to therapy are different. cGVHD and its current treatments have a spectrum of deleterious effects on normal growth and organ development in children. In addition, the impact of prolonged immunosuppression is of particular concern in childhood, a critical time of immunologic development in response to common infections and immunizations.
Data and research focused on cGVHD in pediatrics are limited. Most studies are small and are often grouped into larger adult series. In comparison to adults, relatively small numbers of children and adolescents undergo transplantation.

INTRODUCTION AND PRESENTATION
Burkitt's (BL) and lymphoblastic lymphomas (LBL) are highly aggressive diseases with distinct natural histories and clinical presentations. BL mostly occurs in the first two decades of life and accounts for 1–2% of all lymphomas. Three clinical variants are recognized: endemic BL, which is primarily found in equatorial Africa and Papua New Guinea, sporadic BL, which presents worldwide but is the most common type in Western countries, and immunodeficiency-associated BL, which is associated with HIV infection. There are important clinical differences in these variants (Table 13.1), with endemic BL involving the jaw, orbit and paraspinal regions in half of the cases as well as the mesentery and gonads, while sporadic BL mostly involves the distal ileum, cecum and/or mesentery, and rarely the jaw. When bulky or disseminated disease is present, extranodal involvement of the ovaries, kidney, breasts and/or central nervous system (CNS) may be seen. Clinical presentation in a Berlin–Frankfurt–Munster Group (BFM) series of 152 pediatric patients included advanced-stage (III/IV) disease in 38%, bone-marrow involvement in 33% and CNS disease in 4%. Overall, 27% of the patients in this series presented as acute leukemia and are usually referred to as the L3 subtype of acute lymphoblastic leukemia (ALL) within the French–American–British (FAB) classification. BL infrequently presents in adults, but does occur with increased frequency in patients with HIV infection.
LBL is most commonly a malignancy of T-cell precursor cells, and as such it is identical to T-cell acute lymphoblastic leukemia (T-ALL).

Corporations tender for their own shares for a variety of reasons. Some stock tenders are made for strategic purposes—to prevent a take-over, to raise the market price of the stock, or simply because the stock represents ‘a good investment.’ For discussion of tendering in these situations, see the articles of Ellis [2] and Guthart [1]. In addition, there may be tactical reasons for a stock tender; one such reason is to reduce bookkeeping and shareholder servicing costs. In this instance, the argument runs roughly as follows: “The annual cost of servicing a holding is independent of the number of shares; consequently, the cost per share of servicing small holdings is relatively great. Let us reduce these high per-share costs by buying up small holdings.” Typical procedure is to then mail out an offer to buy holdings of less than a certain size directly, thus permitting the shareholder to dispose of his holding without paying the usual brokerage and odd-lot fees. Frequently no premium is offered except for the avoidance of brokerage fees. If one were to consider the premium offered as a controllable variable, it would be surprising to discover that its optimal value were exactly zero. One also recognizes that the maximum shareholding tendered for may be another decision variable available for optimization. See the appendix for data on tenders of this sort made in recent years. The variety of policies seems to indicate an almost complete absence of systematic application of the ideas presented here.
